Service Request Form

To book and appointment, you can call our office (see above) or complete the service request form on this page, then click “Submit”. Pay careful attention to the required fields to avoid delays in receiving a response from our office. Submissions will be answered via email within 24-48 business hours; so, be sure to add appointments@forgingforward.ca and lisa.bowman@forgingforward.ca to your “safe list” and check your spam folder.

NEXT STEPS: Upon receipt of this website form, our Lead Counsellor or Client Care Coordinator(s) will review the information and contact you in order match you with a counsellor. We will need your consent to send our intake forms via email from our secure practice-management software. We do our best to respond to inquiries within 48 hours and book your first appointment within 7 days of  receipt. This will largely depend on your availability as well as the Therapist’s schedule.
